Straw Ballot Please fill out the straw ballot to nominate any qalified active JLF member for any Board of Director position. Leadership Interest Form Click here to access the leadership interest form.
Press Release Information Please provide us some information for a press release on your recent Jr. League event.
Catalyst This form will provide you a way to give us the summary of your recent League event to have for Catalyst. You do not have to write an article, but please just provide us some basic information.
Member Status Change Form Please use this form to notify the Corresponding Secretary of a change of Name, Address, Telephone Number, or Member Status.
Provisional Membership Form Please use this form to nominate a Provisional for Membership.
"Spark" and "Spirit" of the League Please use this form to nominate a Provisional and Member for "Spark of the League" and "The Spirit of the League".
Member Reimbursement Form Please fill out the attached Reimbursement Form and submit to the Treasurer with the original receipts.
Leave Request Form Please use this form to request a leave of absence.
